The Goldilocks Mastectomy was devised for patients with larger breasts who underwent a mastectomy and desired a simple breast reconstruction without resorting to implants or transferring tissue from distant parts of their body (flaps). Here we see a 67-year-old female with extensive right breast cancer who undergoes a bilateral mastectomy. We use the residual skin and fat after her mastectomy to create her smaller breasts. Next, we see her 5 years after her mastectomy and reconstruction. Her breasts have been completely removed and the only remaining tissue is skin and fat making up her new breast mounds.
This is a 44-year-old female with right breast cancer, who desires a bilateral mastectomy. She refuses an implant-based reconstruction. Because of her obesity and body shape, getting tissue from her abdomen is difficult with high risk of complications. We offer her a Goldilocks mastectomy with free nipple grafts (nipples are removed and replaced as skin grafts) with immediate placement of a flap from the back. She undergoes a second surgery to tighten up her breasts and improve their shape.
This 45-year-old female with right breast cancer proceeds with bilateral Goldilocks mastectomy and free nipple grafts with immediate implants. She requires no further surgery and is very pleased with her result. Her surgery is completed in 2 1/2 hours and she’s discharged home from our ambulatory surgery center and requires no hospital admission. She is shown here six months after her surgery.
This is a 45-year-old female with right breast cancer who desires a bilateral mastectomy and reconstruction. She undergoes bilateral Goldilocks mastectomy with free nipple grafts and immediate implant placement. She completes her cancer surgery and reconstruction in one surgery and is very pleased with her result.
This 47-year-old female with obesity, eczema, and diabetes presents after neoadjuvant chemotherapy for right breast cancer. She desires bilateral mastectomy and reconstruction. Given her risk factors, she is a poor candidate for either immediate or delayed implant or autologous reconstruction. We offer her bilateral Goldilocks mastectomy and free nipple grafts. She heals nicely and we supplement her volume six months later with silicone implants placed above the pectoral muscle.
This 50-year-old female with morbid obesity and right breast cancer undergoes a bilateral Goldilocks mastectomy with free nipple grafts. Six months later, we supplement her volume with silicone implants placed on top of her pectoral muscle. She heals well and is very pleased with her final result. This approach of delayed implant supplementation after Goldilocks mastectomy was first described by Dr. Schwartz in 2017.
